An eyewitness said a car was swerving across the road. The witness tried to flash his lights at the car to get it to stop or slow down.
"I saw somebody drunk all over the road, hit this guardrail head on," witness Steven Farrell said.
The driver lost control on one side of bridge, hit the overpass and knocked part of concrete railing onto the ground.
The witness said he saw the driver get out of the car and run.
Durham police are searching for the driver.
A passenger in the car was taken to Duke Hospital, but their condition is not known.
Authorities have not released any other information about the incident at this time.
