Troopers said Jessie Sanders Jr., 43, of Wendell, was driving an International truck east on the highway when he crossed the center line and collided with the driver's side of a Hyundai van carrying seven people.
The driver of the van, Renee Stokes, 33, of Wilson, was taken to WakeMed where she died.
Angelina Rebollar, 13, of Louisburg, died at the scene.
The pastor of Hope Lutheran Church in Wake Forest told ABC11 that Rebollar, who everyone knew as Angel, was confirmed at the church in May.
"She really was a faithful young lady, and she'll be greatly missed," said the Rev. Lew Upchurch.
Rebollar's grandmother, Debra DeSha, 61, and her 10-year-old sister Crystal are still recovering at WakeMed.
Stokes' three sons, Seth, Elijah, and Isaac, 3 months, 5 and 10, respectively, were injured in the crash. All suffered serious injuries.
Sanders sustained minor injuries.
Troopers said charges are pending.
