Please include the following details regarding your issue or concern:

Program name
Date and time program aired/issue was observed
Description of the issue (e.g., missing captions, captions out of sync, etc.)
If watching via broadcast, specify whether over-the-air or the name of the pay TV provider
If watching online, provide the device (e.g., tablet, smart TV, mobile phone, laptop), device model number, and software/application (iOS/Android, Internet browser, streaming app,) used to view the program
Your name, address, and contact information (email and/or phone number)
