Investigators said a car was headed the wrong way in the northbound lanes of SW Cary Parkway when it collided with a flat-bed truck.
The driver of the car was taken to WakeMed.
The truck driver was taken to the hospital with non-life threatening injuries.
All lanes reopened around 5 p.m.
