The alert was originally issued around 9:20 a.m. Authorities said Evans, who was 85, was last seen Primrose Retirement Home in Angier.
Evans wandered away from the Primrose Retirement Home around 7:40 a.m. Monday.
Investigators asked the state medical examiner Monday to determine how Evans died. Angier's Police Chief Bobby Hallman said Tuesday Evans died from a head injury after falling into a deep ravine. There was no foul play.
Eyewitness News has been told the 85-year-old woman apparently walked out while breakfast was being prepared for the residents. A search began around 9 a.m. and a Sliver Alert was issued.
Police say shortly before 1 p.m., a highway patrol helicopter taking part in that search spotted Evans' body behind a manufacturing plant several hundreds of yards down the road from the assisted living facility.
The facility's administrator declined to comment.
